Looking for any thoughts. Patient today had flare up of back pain over the weekend, some radicular pain into hips and thighs. Patient has tenderness on both sides of the low back. Twist and arm raise, L twist painful, R twist no pain. Do 2 sets of 20 secs, patient has increased pain twisting back to the L to return to neutral. Told her to continue to try twist and arm raise for HEP anyway to see if it does help. Any thoughts since the return to neutral is painful?

I had a pt the other day with similar "both sided" pain with arm raise, twist. however leg raise one side pain, the other none... so I started with leg raise, then went to step up (leg press) and came back to twist, one side did not hurt so I made that the good side, same with arm raise. did you check the other items?

I saw this particular patient for the second visit. She was actually doing much better after starting a steroid pack. She still had some back pain and responded much better to the quicky twist and arm raise. She was able to touch her toes following the quick arm raise and twist without pain.

I have found this a couple of times with some acute cases. I usually keep trying to find "another move". I always tell acute cases to "keep moving"...walk to the kitchen/bathroom whether they have to go or not every 30-40 min.
Love it when I only have to use the big 2 exercises: Twist and arm raise.
