I have been using TMR and concepts for several months. where I get stuck is when both sides hurt. not sure if I need to work the one that hurts less (good side in comparison). also what to do if everything (yes I have pts that ALL 5 both sides hurt) increases pain, or there is no change after doing the whole form (3sets on everything -3data points). any thoughts?

please give me a call and lets talk about a couple of patients that have presented like this. I will share with you a couple of examples on what to do.

I have been using TMR the last month. I have had great success except for one patient. She has been getting great results with all parts of her body except her upper back,traps,C5. On her 6th session she could perform all fab5 with increase ease except for the arm raise. It is worse than ever on both sides. After doing all the fab5 her back and neck were hurting her more than when she started. she says she has sharp pains, and she is very tender to the touch. she has rounded shoulders,Osteo. she gets spasms in her chest. When we started her 6th session she could do arm raises to about 150° after the fab5 she could only raise them to 100°

Lou 33, some things that work with shoulder patients(traps, neck whatever) that may be worth a try. passive arm pull (myofascial type slow sustained pull). have pt supine, move "bad"(or worse) arm into prom flexion until tightness or guarding. have pt "push" good arm into plinth( isometric extension)5sec hold, relax. when good side relaxes, bad arm relaxes...take up the slack on bad arm, repeat. then do abduction the same way with iso adduction on good arm.

maybe she doesnt need to do the upper torso stuff at all-if her arms (and maybe ribcage) dont wanna elevate, maybe they will depress with ex such as leg raise or shift like in bent knee toe reach or even sit-stand...i really like the leg raise or even just a sitting knee to chest move for shoulder elevation issues...and if all sides are bad-maybe you can do them in a varied position (hooklying, supine, side lying, standing, etc) and find something that moves well to unlock the pt...then as their body unlocks and enjoys moving more-then maybe the fab 5 will be ok to progress towards
